Tutoring Services Agreement

This Agreement is made on date of purchase between Empower Educational Consulting Services AND the Student / Parent or Guardian purchasing services.

1. Services Provided

The Tutor agrees to provide tutoring services in the following subjects:

Tutoring sessions will occur on the following days and times weekly/monthly:

Sessions will be held at: On Website needs to say Student’s home, Chosen location by tutor, or online.

4. Payment

The Student/Guardian agrees to pay the Tutor:

$50 per session

-Each session lasts ONLY 45 minutes

-Each session must be paid for in advance.

-Payments can be made by Zelle, Venmo, or PayPal.

Payment due 48 hours before session.

Payment needs to include child’s name, session dates/times

Payment should include $15 travel fee if session will be at student’s home.

5. Cancellation Policy

Cancellations must be made at least 48 hours in advance.

If a session is canceled without proper notice, the Tutor reserves the right to charge for the full session.

-If the tutor needs to cancel an appointment due to unforeseen circumstances, the customer will be able to reschedule that session, however no refunds will be given.

6. Responsibilities

The Tutor will prepare the appropriate materials and will try their best to help each student succeed, however, the Tutor makes no promises or warranties with regards to a student’s performance as a result of any tutoring provided.

The student is responsible for being present for each scheduled session, participating in each activity, and behaving in a respectful manner at all times.

7. Termination

Either party may terminate this Agreement with at least a two-week written notice.

8. Confidentiality

Both parties agree to keep all information regarding tutoring sessions and student performance confidential.

9. Attendance

The student must show up on time for their session. Each session will only last 45 minutes and the amount of minutes that a student is late will be taken away from the time of the session. For example, if a student is 15 minutes late, their session will only last 30 minutes.

If a student is late for three sessions in a row, the tutor will no longer be able to serve them and any remaining balance owed to the client will not be returned.

10. Acceptance

By signing below, both parties agree to the terms outlined in this Agreement.
